Trip Declaration Form
This form can only be collected from the Union Office should be returned in person to the Sports Union Administrator.
The form must be submitted not less than one day before the Trip and can only be submitted in person by the Driver.
Failure to submit this form prior to any trip will result in the Club being subject to a £50 fine and no expense claims being processed for the trip.
