Purchase Order Form > Download
A Purchase Order Request Form can only be completed by the Club Treasurer or Club President and it must be completed accurately before it will be processed.
Clubs must have sufficient funds in their account to put through a purchase order request.
Once a purchase order request is received by the Sports Union, it will be checked, and only when the Sports Union is satisfied, an official Purchase Order Form will be sent to the supplier.
Personalised kit can’t be paid for through membership monies, individual members must pay for this themselves. Once each individual has paid the required sum of money into the club private account the order will be processed.
