Club Affiliation forms for 2011/12 will be given out to all Club Presidents at the first Clubs Council meeting in September. We have put these documents here so you can have a look at them, and, once we've given them out in September, you can download additional copies if you misplace your own copies.
Please do not hand any of these forms into the Sports Union until you have been instructed to do so, as they will not be accepted and you will be asked to return them at a later date.
